By registering, the participant agrees to the following terms and conditions.

Assumption of Risk & Liability Waiver

I acknowledge that participation in Utah Islanders training sessions, camps, practices, games, and related activities involves physical exertion and inherent risks, including the risk of serious injury or death.

I certify that I am in good physical condition and able to safely participate in these activities.

In consideration for being allowed to participate, I voluntarily assume all risks associated with my participation and hereby release, waive, and discharge the Utah Islanders, its directors, officers, employees, coaches, volunteers, agents, and affiliates (collectively referred to as the “Released Parties”) from any and all liability, claims, demands, damages, or causes of action arising from injury, illness, loss, or death that may occur as a result of my participation, whether caused by negligence or otherwise.

Media Release

I grant the Utah Islanders and its representatives the unrestricted right to photograph, film, and record my participation and to use my name, likeness, voice, and appearance in any media for promotional, advertising, educational, or commercial purposes without compensation or further approval.
